To create and keep a good volleyball team is even harder than you thing, but once you have it it would give excellent results and can be successful, it is need it to take care of many aspects and always have clear all the points of the work they are going to do, the formation is equally hard as keeping it working, first is to make everybody to be "connected" to one idea and proceed from there, is like having a new business, first you need to now what are you going to do on and then work based on that idea.
The head of the team always going to be the coach, he/she has the bigger responsibility of all why? Well is the person who decides which players are chosen to be part of the team and which should leave or should be on the team for several reasons, basically those who don't respect the idea and work plan of the team, and basically is the one in charge of training in a good way the team and tell them what to do. How to manage them.
At what moment are the problems coming?
A volleyball team as every other sport team or organization needs an order, rules and goals, those are basic aspects to make it work, for example the order is the idea to know, coach is who takes the decisions, for example who plays in a game and who does not and this situation works with the rules, if a player is mad because didn't play a game he/she should know the rules, like if the coach did not need him/her to play but need that player to help the confidence of the teammates form the bench it's something the he/she needs to respect.
About goals
Goals are made to point the work to somewhere, known that all the effort is made to accomplished something, coach and players decide the goals depending on the circumstances where the team is, if they have the resources to be champions that could be a goal or if it's a new young team, probably the goal should be to make a process of a couple of seasons and try to get experience from other players, situations and competitions try to have as good results as it's possible.
So its necessary that everybody knows perfectly which is the place of each person on the team, not just to avoid problems but to contribute with the aspects I wrote before, all are part and have responsibility for what is going on inside the team and I say inside because there are other outside aspects that are out of the control of the group and that is way it should be an united and well managed.
